The following is an itemized list of hazardous materials violations that have not been <br /> addressed for R&B Pallets Sales And Repair as of October 12, 2020. <br /> Open violations from July 23, 2020 inspection <br /> Violation #1 - Failed to pay annual operating fees. <br /> Facility failed to pay annual operating fees for the year 2020. Fees shall be paid once a year at the time of submittal <br /> of an inventory form. Immediately pay the annual fee to the EHD Accounting Office located at 1868 E Hazelton <br /> Avenue, Stockton, CA 95205. Fee may also be paid online at www.sjcehd.com(Convenience fee may apply). <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#2 - Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS) 2018, 2019, and 2020. As of January 1, 2013, all businesses were required to submit all business plans <br /> electronically. Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, <br /> and submit to the EHD for approval. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#3 - Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented for 2018, 2019, and 2020. A <br /> Hazardous Materials Business Plan shall be established and implemented for emergency response to a release or <br /> threatened release of a hazardous material when storing hazardous materials at or above the thresholds quantities <br /> of 55 gallons for materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard <br /> temperature and pressure.
