(7-Eleven) to document the destruction of four onsite groundwater monitoring wells (MW-1 <br /> through MW-4) and two onsite dual-nested remediation wells (SPV-1 and SPV-2) located at 853 <br /> East Yosemite Avenue in Manteca, California (Figures 1 and`2). The report has been prepared <br /> in response to a California Regional Water Quality Control Board (CRWQCB) letter dated June . <br /> 19, 2008, requesting that existing monitoring wells onsite be destroyed prior to case closure <br /> (Attachment A). <br /> j <br /> Well Destruction <br /> Perm ittinglUtility Clearance - Stantec obtained well destruction permits from the San Joaquin <br /> Environmental Health Division (SJEHD) (Attachment B). Prior to'conducting subsurface work <br /> at the site, Underground Service Alert (USA) was contacted to delineate subsurface piping near <br /> the site with surface markings. In addition, a private utility locator service was used to delineate <br /> subsurface utilities at the site. <br /> Health and Safety - Stantec modified our existing site-specific Health and Safety Plan for the <br /> scope of work as required by the Occupational Health and Safety Administration (OSHA) <br /> Standard "Hazardous Waste Operations and Emergency,i Response" guidelines (29 CFR <br /> 1910.120). The document was reviewed and signed by SECOR personnel and subcontractors <br /> prior to performing work at the site. <br /> h <br /> Well Destruction - On July 15, 2008, Stantec supervised the destruction of four onsite <br /> groundwater monitoring wells (MW-1 through MW-4) and two onsite dual-nested remediation <br /> wells (SPV-1 and SPV-2) by Gregg Drilling and Testing, Inc of Martinez, California (Figure 2 and <br /> Table 1).
