FREMONT ST SR0081051 <br /> STOCKTON , CA 95203 <br /> The San Joaquin County Environmental Health Department ( EHD ) has completed the review of <br /> the application for a permit to replace the two 87 and , 91 gasoline drop tubes with new OPW- <br /> 71SO overfill prevention valves on the underground storage tank ( UST) system at the above <br /> noted address . The permit has been approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control <br /> District for any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work . All new addenda supersede the previous ones . <br /> 3 . Contact Paul Nso at (209 ) 468- 3432 to arrange the following appointments (a 48 - hour <br /> notice is required for inspection appointments ) . <br /> • Overfill prevention verification : Measurement of drop tubes to verify 95 % tank volume <br /> shut off. Verification of flapper functionality . Ensure that all pertinent tank charts and <br /> calculations are available for review at the time of inspection . <br /> • Verification that ball float (s ) , if installed , do not interfere with the installed flapper valve . <br /> • If concrete will be broken near the 87 , 91 tanks , a secondary containment <br /> test must be performed on these components once concrete is poured in accordance <br /> with the manufacturer' s guidelines and test results submitted to the EHD within 30 days <br /> of testing . <br /> Please be aware that the facility cannot receive fuel in the 87 and 91 UST system <br /> prior to the EHD inspection and approval. <br /> 4 . Any individual performing the work of a service technician must meet the following <br /> qualifications : <br /> a . Possess or be employed by a person who possesses one of several permissible <br /> valid , current contractor' s licenses issued by the Contractors State License Board <br />
