Overfill Prevention Equipment Inspection Report Form using most recent data. Fill out this new form using data <br /> from the most recent UST Monitoring System Certification Report Form (for A/V only or Ball Float&A/V) and/or <br /> most recent BP-specific annual facility inspection form (for Ball Float&A/V or Fill Tube Valve). For example, if <br /> the last Monitoring Certification for a site was done on 1/1/2018, use the data from the 1/1/2018 Monitoring <br /> Certification Report Form to populate the Overfill Prevention Equipment Inspection Form and date it 1/1/2018. <br /> c. Neither of the above. Overfill Prevention Equipment Inspection Report Form by having testing contractors <br /> perform overfill equipment inspections at all of our sites between October 1, 2018 (when the regulations and <br /> henceforth the new Form came into effect) and October 13, 2018 (deadline for first inspection). As you can <br /> imagine,this timeframe will present a challenge for us due to constraints on contractor availability and <br /> resourcing.
