Maintenance and monitoring records for the last three years were not found on site . - Previous alarm logs of the monitoring panel were not found . The most recent alarm log is dated 6 -7 -2019 . -Alarm logs are not being maintained for the fuel alarms on site : L1 piping sump on 2/26/ 19 L3 dispenser on 2/ 14/2019 and 12/ 16/2018 These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; ( 3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; (5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD . This is a Class II violation. 204 CCR 2712 ( i ) Current monitoring plan approved by the EHD not found on site . An approved copy of the monitoring plans were not available for inspection . A copy of these plans shall be accessible on site at all times . Immediately locate and retain a copy on site . This is a Class II violation . 205 CCR 2712 ( i ) , 2634 (e) Failure to submit a current UST Response Plan and have it available onsite . An approved copy of the response plans were not available for inspection . A copy of these plans shall be accessible on site at all times . Immediately locate and retain a copy on site . This is a Class II violation . 209 CCR 2716 ( a-e ) Failure to conduct the designated UST operator visual inspection at least once every 30 days . A designated operator inspection was performed on June 6 , 2018 and the next inspection was performed on June 91 20191 1 year later. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection . Ensure that all designated operator inspections are performed within the required time frames . This is a Class II violation . 210 CCR 2716 (f) DO monthly reports not maintained on site for 12 months or for 36 months (after October 1 , 2018 ) , The 2018 designated operator monthly inspection reports were not found on site . The most recent inspection report on site is dated 6 -26 - 18 . An alarm history dated 6-6 -2019 was observed on site but an inspection report was not found . Designated operator monthly inspection reports for the previous twelve months shall be retained on site . Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site . Submit copies to the EHD . This is a Class II violation .
