Present at the meeting were you,Margaret Lagorio and
Jeffrey Wong representing PHS/EHD,and Bob Marty and Tim Cuellar from Advanced Geo-
Environmental,Inc. (AGE). This letter summarizes the discussions at the meeting.
You explained that you were unable to meet PHS/EHD deadlines to begin commencement of the work
proposed in Work Plan Addendum-August 1999 dated August 10, 1999 prepared by AGE due to
financial difficulties. You have been notified by the State Water Resources Control Board Cleanup
Fund(Fund) of the acceptance for the placement of the above referenced site on the Fund's Priority List
B in a Notice of Eligibility dated November 8, 1999. You stated that the Fund required deductible plus
additional monies have already been expended in investigating the site. It was discussed that there are
no known nearby wells that may be impacted by the documented MTBE contamination so the further
investigation required is not of an emergency nature. PHS/EHD and you agreed that the following
schedule would be followed:
1) Within one week after receipt of the issuance of a Letter of Commitment (LOC) from the
Fund, you will submit a reimbursement request to the Fund for the work that has already been
performed at the site.
2) The current proposed work will be initiated within one month of receiving reimbursement
from the Fund for the previous work performed. (The current work plan should be submitted
to the technical staff of the Fund for cost preapproval now.)
3) A report of the work to be performed will be submitted to PHS/EHD within 6 weeks after the
completion of the first phase of work(installation of monitoring wells and analyses of the well
water samples) is completed.
If you have any question, please contact Jeffrey Wong at (209) 468-0335.
Donna Heran,REHS,Director
Environmental Health Division
Jeffrey Wong,REHS Margaret gor of ,RENS
LOP/Site Mitigation Unit IV Supervisor
c: RWQCB, Central Valley Region-Marty Hartzell
c: Advanced GeoEnvironmental Inc. -Tim Cuellar
A Division of San Joaquin County Health Care Services
