(AGE), an application for a well permit, and a <br /> permit fee were received by San Joaquin County Public Health Services, <br /> Environmental Health Division (PHS-EHD) on May 14, 1998. The work plan was <br /> approved with conditions by PHS/EHD on June 22, 1998. The proposed work <br /> has not commenced to date. <br /> PHS/EHD has been informed that you have not completed an application for <br /> reimbursement from the State Water Resources Control Board Underground <br /> Storage Tank Clean Up Fund (The Fund).
