A proposal for initial assessment and/or remediation for the above referenced site has been submitted to this office. All proposals and reports submitted must be accompanied by a Letter of Acknowledgement from the responsible pa ty(ies) which states, at a minimum, the following: I (We) declare, under penalty of perjury, that the information and/or recommendations contained in the attached proposal or report is/are true and correct, and that all work and reports which required geologic or engineering evaluations an /or judgements have been performed under the direction of an appropriately registered or certified professional. The attached proposal or report utilizes the "Regional Board Staff Recommendations For Initial Evaluation and Investigation of Underground anks" and . "Appendix A" reporting format.
