According to Adriana, Dollar Tree corporate required these wipes to be pulled off <br /> the shelves and not to be sold until further notice.The cardboard box was dated 1-28-14. <br /> Any person who generates a waste shall determine if the waste is a hazardous waste. <br /> Immediately make a hazardous waste determination for the used printer ink cartridge and multi-surface wipes, and <br /> manage them according the Title 22 hazardous waste regulations. <br /> Submit a statement and supporting documentation explaining how this waste was managed. <br /> This is a Class II violation. <br /> 104 CCR 66262.34(d)(2)No modified contingency plan. <br /> The contingency plan posted near the telephone in the store office(IMG 6468, 6470-3), is lacking an emergency <br /> coordinator, and the hazardous waste emergency information sheet(IMG 6469), also posted in the store office, listed <br /> William Flores as the emergency coordinator.According to Adriana Montoya,Asst. Operations Manager,William no <br /> longer works at the facility. <br /> There must be at least one emergency coordinator on site or on call (i.e., available to respond to an emergency by <br /> reaching the facility within a short period of time)with the responsibility for coordinating all emergency response <br /> measures. This emergency coordinator shall be thoroughly familiar with all aspects of the facility's contingency plan, <br /> all operations and activities at the facility,the location and characteristics of waste handled,the location of all records <br /> within the facility, and the facility layout. In addition,this person shall have the authority to commit the resources <br /> needed to carry out the contingency plan.Additionally,the following information must be posted by a phone:the name <br /> and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, <br /> fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. <br /> Immediately appoint an emergency coordinator and post the required information by a phone. <br /> Adriana Montoya,Asst.
