Operations Manager,were not sure if the ink cartridge would be picked up by the hazardous waste hauler. 2.An open cardboard box filled with packs of Multi-Surface pre-moistened wipes was found in a corner of the store's back storage area (IMG 6459-63). According to Adriana, Dollar Tree corporate required these wipes to be pulled off the shelves and not to be sold until further notice.The cardboard box was dated 1-28-14. Any person who generates a waste shall determine if the waste is a hazardous waste. Immediately make a hazardous waste determination for the used printer ink cartridge and multi-surface wipes, and manage them according the Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was managed. This is a Class II violation. 104 CCR 66262.34(d)(2)No modified contingency plan. The contingency plan posted near the telephone in the store office(IMG 6468, 6470-3), is lacking an emergency coordinator, and the hazardous waste emergency information sheet(IMG 6469), also posted in the store office, listed William Flores as the emergency coordinator.According to Adriana Montoya,Asst. Operations Manager,William no longer works at the facility. There must be at least one emergency coordinator on site or on call (i.e., available to respond to an emergency by reaching the facility within a short period of time)with the responsibility for coordinating all emergency response measures. This emergency coordinator shall be thoroughly familiar with all aspects of the facility's contingency plan, all operations and activities at the facility,the location and characteristics of waste handled,the location of all records within the facility, and the facility layout. In addition,this person shall have the authority to commit the resources needed to carry out the contingency plan.Additionally,the following information must be posted by a phone:the name and phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an emergency coordinator and post the required information by a phone. Adriana Montoya,Asst. Operations Manager, listed Dineen Goodwin, Store Manager, as the emergency coordinator on the hazardous waste emergency information sheet at the time of inspection. This was corrected on site.
