Yosemite Ave <br /> Manteca, CA 95337 <br /> Dear Inspector Florido, <br /> In response to the inspection conducted on May 24, 2019, and amended June 27, 2019, at Store <br /> #01238 in Manteca, California, please see the outline below, and the exhibits which follow, <br /> referencing existing procedures, documentation and/or actions taken: <br /> • The Company has a trash assessment program in place—the "Yellow Dot" stickers. The <br /> Store Manager or an Assistant Store Manager must examine each bag or box of trash to <br /> determine that no hazardous waste is being disposed of improperly. Whomever inspects <br /> the trash places a yellow sticker on the bag or box with their initials and the date of <br /> inspection. (The process is also described in our quick reference training guide.) <br /> • The Store Manager's contact information was added to the Emergency Contact List and <br /> posted by a phone for access by all Associates (EXHIBIT A). <br /> • An Environmental Health& Safety Specialists retrained the Store Manager and three <br /> Assistant Store Managers on the hazardous waste program and related emergency <br /> response procedures on June 28, 2019 (EXHIBIT B). A copy of the training guide used is <br /> provided as an email attachment. <br /> • Generator manifests are submitted to DTSC on the Company's behalf by our hazardous <br /> waste vendor Stericycle (EXHIBIT Q. However, generator copies of the manifests cited <br /> in the report were also resubmitted to DTSC by Dollar Tree (EXHIBIT D). <br /> The signed certification is also included.
