After October 1st, 2018 <br /> Designated operator monthly inspection reports for the previous thirty-six months shall be retained on site. Locate <br /> and ensure that copies of the previous twelve months of designated operator monthly inspection reports are <br /> maintained on site. Submit copies to the EHD. <br /> This is a repeat violation, Class II. <br /> 215 CCR 2712(i) Failure to retain a copy of the permit to operate at the facility. <br /> A copy of the UST operating permit was not found on site. A copy of the UST operating permit and all attachments <br /> and conditions shall be retained at the facility. Immediately obtain a copy of the permit and retain it on site. <br /> This is a repeat violation, Class II. <br /> 306 CCR 2630(d), 2641(a)Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The 91 product fill sump sensor was laying on its side,the 87 product fill sump sensor was tilted approximately 45 <br /> degree angle, UDC 9/10 had a 208 sensor,the bottom of the UDC was not flat and the sump sensor was not at the <br /> lowest point of the UDC. The sensors listed above were not located to detect a leak at the earliest opportunity. <br /> Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The <br /> sensors were relocated to the lowest point of the sump by the service technician. Ensure that all monitoring <br /> equipment is maintained to detect a leak at the earliest opportunity. <br /> This is a repeat violation, Class II. <br /> 315 HSC 25291(e)Water in secondary containment not removed, analyzed, and properly disposed of(pre-Jul 2003). <br /> Liquid was observed in the UDC 9/10 sump. If water could enter into the secondary containment by precipitation or <br /> infiltration, it must be removed and disposed of properly. The service technician removed approximately one gallon <br /> of liquid from the UDC 9/10 sump.
