Mr. Price was unable <br /> to produce other forms such as Land Disposal Restrictions, employee training records, manifests for years <br /> prior to 2000 (as detailed in the attached SOV). <br /> b. Contingency Plan: <br /> The contingency plan was outdated and did not list the name of the current emergency coordinator. The plan <br /> also failed to indicate the emergency coordinator's phone number and the facility map in the contingency plan <br /> did not clearly indicate the location of the PBR permitted treatment unit and the hazardous waste 90-day <br /> DTSC (3/95) Page 1 of 5 <br />
