SAN JOAQUIN COUNTY <br /> ENVIRONMENTAL HEALTH DEPARTMENT <br /> 1868 East Hazelton Avenue, Stockton, California 95205-6232 <br /> Telephone:(209)468-3420 Fax: (209)468-3433 Web:www.sigov.org/ehd <br /> CONTINUATION FORM Page: 3 of 5 <br /> OFFICIAL INSPECTION REPORT Date: 04/07/14 <br /> Facility Address: 6801 Longe Street, Stockton, CA Program: CECL <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II,or MINOR-Notice to Comply) <br /> Onsite to conduct a routine hazardous waste CECL (conditionally exempt commercial laundry) <br /> facility inspection. Consent for inspection, sampling, and photographs was granted by Mr. Deniz <br /> Garrett, the assistant plant manager. Inspection was performed on April 7, 2014, the checklist of <br /> findings was provided the same day; the full inspection report was mailed to the facility on April <br /> 9, 2014. <br /> 201. No comprehensive contingency plan was available onsite at the time of inspection. Each owner or <br /> operator shall have a contingency plan for the facility. The contingency plan shall be designed to <br /> minimize hazards to human health or the environment from fires, explosions, or any unplanned sudden <br /> or non-sudden release of hazardous waste or hazardous waste constituents to air, soil, or surface water. <br /> A site specific contingency plan shall include all of the following elements: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, <br /> soil, or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, cleanup <br /> contractors, and state & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator and any <br /> alternates <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decontamination), <br /> including location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including at least two routes and signals to be used <br /> -telephone number of Cal CIES Warning Center <br /> This information must be kept current at all times. Immediately prepare and implement the contingency <br /> plan and submit evidence of implementation to the EHD by May 9, 2014. <br /> 215. No apparent hazardous waste management training is occurring at this facility, and no records <br /> pertaining to this specific training were available for review at the time of inspection. Facility personnel <br /> shall complete a training program that teaches them to perform their duties in a way that ensures the <br /> facility's compliance with Title 22 regulations. This program shall be directed by a person trained in <br /> hazardous waste management procedures, and shall include facility personnel's instruction on <br /> hazardous waste management procedures relevant to the positions in which they are employed, and <br /> shall specifically include the following training elements: <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Elena Manzo (209) 953-7699 <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
