This <br /> should include the company name, address, EPA ID#, Unit#,Tier of Authorization, and Date of closure. <br /> If you plan to continue to use/begin use of the Drum Rinsing/Crushing unit,then I would require that you resubmit the <br /> following attached forms at least 60 days before starting use of the unit.
