Duration of noncompliance, including exact dates and times of noncompliance, and if the noncompliance <br /> continues, the time by which compliance is reasonably expected to occur. <br /> c. All measures taken or to be taken to reduce, eliminate, and prevent recurrence of such an upset, slug, accidental <br /> discharge, or other conditions of noncompliance. <br /> d. All accidental discharge reports shall be submitted to the City of Lodi, Public Works Director, 221 W. Pine Street, <br /> Lodi, CA 95240. <br /> PART 4- REOPENER CLAUSE <br /> A. This permit may be reopened and modified to incorporate any new or revised requirements resulting from the City <br /> of Lodi's reevaluation of its local limits. <br /> B. This permit may be reopened and modified to incorporate any new or revised requirements developed by the City <br /> of Lodi as are necessary to ensure POTW compliance with any new federal, state, or local regulations promulgated. <br /> PART 5-COMPLIANCE SCHEDULE <br /> Not Applicable. <br /> PART 6-SPECIAL CONDITIONS <br /> A. Discharge must be adjusted to a pH of 6 to 10. <br /> B. Samples for regulated pollutants shall be taken once ever six months. <br /> C. Annually Total Toxic Organic samples must be grab samples and be analyzed by EPA method 624 and 625. <br /> PART 7-APPLICABLE PENALTIES <br /> Penalties which may be assessed by the City for discharge or permit violations include: Notice of violation, Cease <br /> and Desist Order, Discharge Permit suspension or revocation, Compensation to the City for damages to POTW, civil <br /> penalties not to exceed six thousand dollars for each day in which such violation occurs, and termination of service. <br /> PART 8-STANDARD CONDITIONS <br /> See attached sections of City of Lodi Municipal Code Chapter 13.12 <br />
