Hazelton Ave <br /> Stockton, CA 95205 <br /> Sent to Elianna Florido at eflorido@sjgov.org <br /> Re: Dollar Tree#05531 <br /> 2400 McHenry Ave <br /> Escalon, CA 95320 <br /> Dear Inspector Florido, <br /> In response to the inspection conducted on May 24, 2019, and amended June 26, 2019, at Store <br /> #05531 in Escalon, California, please see the outline below, and the exhibits which follow, <br /> referencing existing procedures, documentation and/or actions taken: <br /> • The Klar& Danver foaming hand soap found above the mop sink was added to the <br /> STOU tote (EXHIBIT A). <br /> • Contact numbers for the emergency coordinator and backup coordinator were added to <br /> the Emergency Contact list and posted for access by all Associates (EXHIBIT B). <br /> • An Environmental Health& Safety Specialists retrained the Store Manager and three <br /> Assistant Store Managers on the hazardous waste program and related emergency <br /> response procedures on July 9, 2019 (EXHIBIT C). A copy of the training guide used is <br /> provided as an email attachment. <br /> • All generator manifests and final manifests are submitted to DTSC on the Company's <br /> behalf by our hazardous waste vendor Stericycle (EXHIBIT D). However, generator <br /> copies of the manifests cited in the report were also resubmitted to DTSC by Dollar Tree <br /> (EXHIBIT E). <br /> • Regarding the "Pull &Hold"items, the USB cables were processed as universal waste <br /> and placed in the STOU tote. All of the lights were various styles of night lights which <br /> were processed as hazardous waste and also placed in the STOU tote.
