This violation has been corrected. Violation corrected on 10-31-19. The modified contingency plans were completed and posted in the office, in the break room and at the hazardous waste rack. In addition, a map of the store was posted indicating the location of fire extinguishers, spill control equipment kits, emergency exits. The modified contingency plan was posted immediately after we received the plan from the inspector. Pictures of the posted modified contingency plan and the map are included with the response. Violation corrected on 10-31-19 The store manager Sarah Bonilla was listed as the Emergency Coordinator, the District Loss Prevention Manager, Luis Banuelos, was listed as an alternative, and the final alternative emergency coordinator is the corporate contact, Darryl Dinson. (See pics for violation #4.) Violation #106 - Failed to train employees on waste handling and emergencV procedures. This violation has been corrected. Violation corrected 11-14-19. All store associates were required to retrain in hazardous waste handling via the company online training platform. Environmental Compliance Manager, Darryl Dinson, visited the store to retrain the Store Manager on proper completion of the hazardous waste labeling and the maintaining of hazardous waste manifests, utilizing a physical review of the rack, the Hazardous Waste binder, and hazardous waste self-inspection form as a guideline. In addition, associates were advised of the information on the modified contingency plan and maps posted in the office, the hazardous waste accumulation rack and in the break room. The sign off for the training was included as an attachment. We recently changed hazardous waste transporters from Stericycle to US Ecology, as a result we were in the process of updating the training video. We normally conduct our annual retraining in the beginning of the year. A copy of the hazardous waste self-inspection form and a link has been included to the training video.
