Time In: 10:27 am <br /> Time Out: 11:18 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: BURGER KING#14321 Date: 07/29/2020 <br /> Address: 6361 W HWY 12 , LODI 95240 <br /> Owner/Operator: SUNNY GHAI Telephone: (510)333-7802 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052338 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ORDERED FOOD FROM THIS FACILITY ON 7/25/2020 AROUND 1:00 PM AND ALLEGES THEY <br /> BECAME ILL WITH NAUSEA AND STOMACH PAINS AT AROUND 10:00 PM. COMPLAINANT ALLEGES THEY STILL <br /> FEEL A LITTLE BIT OF NAUSEA. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> I went to the facility and spoke to the District Manager and the Manager. <br /> They were not aware of a compliant prior to my inspection. <br /> There have been no ill employees for the last two weeks. <br /> I observed the walk-in refrigerator all food items are stored properly. <br /> I observed food items on the prep line. <br /> The time stickers on the tomatoes and the cheese are within the time. <br /> The temp of the whopper patty in the universal hot hold container is at 158F <br /> and the junior whopper patty in the universal hot hold container is at 187F. <br /> Per District Manager the hold time for the Whopper patties is 30 minutes and the Whopper Jr patties is 45 minutes. <br /> When the hold time is met the product is disposed. <br /> I also observed the"Quality Checks"time log for 7/25/2020 <br /> Hands are washed every 30 minutes or as needed when employee is preforming a specific task. <br /> I observed proper hand washing during this investigation.All hand sinks are supplied with warm water and soap, kitchen hand <br /> sink has paper towels and restroom have functional air dryers. <br /> The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br /> Safety Code.If a reinspection is required,fees will be assessed at the current hourly rate. <br /> Received by: Name and Title: Vishal, District Manager, Sig not capt <br /> EH Specialist: STEPHANIE RAMIREZ Phone: (209)468-9851 <br /> FA0013920 C00052338 SCO04 07/29/2020 <br /> EHD 16-23 Rev.06/30/15 Page 1 of 1 Food Program Complaint Inspection Report <br /> 1868 E.
