The violation was corrected during the inspection. Ariel Canales stated that he would no <br />longer give filters back to his customers. No further action is required to correct this violation. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection report was provided to the facility operator on the day of inspection. The report was amended by <br />the EHD on December 19, 2019. The amended report replaces the initial report. Violation 201 was removed <br />and violation 608 was added. Violation 608 was marked corrected on site. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by January 5, 2020. <br />The amended inspection report was emailed to Ariel Canales, Owner, at lspro.ariel@gmail.com>. <br />Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
