SAN JOANN <br />Environmental Health Department <br />AM LADED <br />8/20/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />1 <br />Date: <br />Dollar General #14661 <br />1940 McHenry Ave. Escalon <br />Au ust 14, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />106 <br />CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />Based on the types of violations observed, the facility employees are not trained on proper hazardous waste (HW) <br />handling or the training provided has been inadequate. <br />1. The following waste items were found bagged and inside a HW tote labeled as Toxic HW (IMG 8287-8293, 8300-4): <br />- A Clairol Frost & Tip hair coloring kit, which the hand held scanner stated should be placed in the Toxic HW tote. <br />However, the hair coloring kit package listed highlighting powder, developing creme, and CC Colorseal Conditioner, <br />each of which is made up of a different list of ingredients. Only the container of developing creme was found inside the <br />kit. According to the package label, the highlighting powder contains sodium metasilicate, which according to the DG <br />HW guidance poster should be placed in the Alkaline HW tote. The developing creme contains hydrogen peroxide, <br />which according to the DG HW guidance poster should be placed in the Oxidizer HW tote. The CC Colorseal <br />Conditioner contain alcohols, fragrance, and oil, which according to the DG HW guidance poster should be placed in <br />the Flammable HW tote. David placed the developing creme into the Oxidizer HW tote at the time of inspection. <br />- A nearly empty and leaky container of Cranberry Premium (liquid) Hand Soap, without any spill clean-up material to <br />account for the missing liquid. David Barajas, Store Manager, did not know what happened to the liquid that was once <br />in the container. According to David, when a product is used by employees to clean the facility or "store -used," an <br />employee will mark the item with a code that is generated once the item is taken out of the store's inventory. The <br />Cranberry hand soap was not marked with such a code, while a nearly empty container of Aloe Cream Soap was <br />marked with a "store -use" code. <br />- A nearly empty container of Palmolive Ultra Dish Liquid soap, without any spill clean-up material to account for the <br />missing liquid. The container was not marked with a "store -use" code. David did not know what happened to the liquid <br />that was once in the container. <br />2. When asked how cardboard boxes, which become contaminated from leakage or spillage of a chemical product, <br />are managed, David stated the cardboard is recycled. <br />The generator must ensure that all employees who handle hazardous waste are thoroughly familiar with proper waste <br />handling and emergency procedures. <br />Provide proof of re-training and training materials to the EHD for employees whose responsibilities include hazardous <br />waste. <br />This is a Class II violation. <br />FA0022207 PR0540012 SCO01 08/14/2019 <br />EHD 22-01 Rev. 09/27/2018 Page 6 of 9 Small Quantity Hazardous Waste Generator OIR <br />1868 E.
