The following is an itemized list of hazardous waste violations that have not been <br />addressed for CHAVEZ TIRES as of September03, 2019. <br />Open violations from July 18, 2019 inspection <br />Violation #303 - Failed to maintain all communication or alarm systems, spill control, or decontamination <br />equipment. <br />The secondary containment for the three 55 gallon drums of used oil had approximately an inch of oil and debris in <br />the bottom of it and the drain was plugged with a rag. All facility communications or alarm systems, fire protection <br />equipment, spill control equipment, and decontamination equipment, where required, shall be tested and maintained <br />as necessary to assure its proper operation in time of emergency. Immediately begin this practice. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #403 - Failed to keep hazardous waste containers closed except when adding or removing <br />hazardous waste. <br />One black metal 55 gallon drum of used oil was observed with an open bung hole on the west side of the property. <br />All hazardous waste containers shall be closed at all times except when adding or removing waste. Immediately <br />close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. <br />❑ This violation was corrected ❑ This violation will be corrected by (date): <br />❑ Supporting documents included <br />Describe actions taken or will be taken to correct violation: <br />Violation #601 - Stored hazardous waste onsite greater than 180 days. <br />Three 55 gallon black metal drums of used oil were observed on the west side of the property without accumulation <br />start dates. The drums contained approximately 80 gallons of used oil total per Arturo Chavez, Owner. Facilities <br />who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site <br />at any time may store waste on site up to 180 days.
