Time In: 9:18 am <br /> Time Out: 9:30 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: Date: 07/30/2020 <br /> Address: 6421 CAPITOL AVE, LODI <br /> Owner/Operator: Telephone: <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00052141 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> THE CHEVRON GAS STATION BATHROOM IS OUT OF SERVICE.THE CUSTOMER WAS TOLD BY THE CLERK TO <br /> USE A DIFFERENT BATHROOM OR TO DO HIS BUSINESS BEHIND THE GAS STATION AT A WALL.THE AREA IS <br /> VERY UNSANITARY, FILTHY,AND SMELLS DISGUSTING. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe.
