SAN JOAQUIN _JuNTy <br />Environmental Health Department <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />PetSmart #1188 <br />Facility Address: <br />10520 Trinity Pkwy, Stockton <br />Date: <br />November 20, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # Remarks <br />106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented) that <br />employees who handle hazardous waste were properly trained, based on the following observations: <br />According to the hazardous waste training guide, the hazardous waste storage area weekly inspection checklists <br />for the past three years are to be maintained in the training and compliance binder (see photo IMG 9502). <br />Completed weekly checklists were not found at the time of inspection (IMG 9487). According to Ashley Hammick, <br />Assistant Store Leader, weekly inspections of the hazardous waste storage area have not been conducted. <br />A Toxic Hazardous Waste tote, containing a partially empty bottle of Skout's Honor Urine Destroyer liquid, was <br />missing the generator name, the accumulation start date, and did not have the physical state and contents boxes <br />checked on the hazardous waste label (IMG 9463-4). <br />A Toxic Universal Waste tote, containing two Nano Basking Spot Lamps, was missing the generator name, the <br />accumulation start date, and did not have the contents boxes checked on the hazardous waste label (IMG <br />9459-62). <br />At the time of inspection, Ashley provided a training record showing the names of current employees and the <br />date when each employee last completed the Hazardous Waste Disposal Program Acknowledgment test. A <br />majority of the employees listed completed the test in 2017 and 2018. <br />REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with <br />proper waste handling and emergency procedures, relevant to their responsibilities during normal facility operations <br />and emergencies. <br />CORRECTIVE ACTION: Immediately train the employees.
