SAN JOAQUIN <br />Environmental Health Department <br />AMErIMM <br />11/18/2019 <br />Small Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />PetSmart #1601 <br />261 Spreckels Ave. Manteca <br />November 14, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # Remarks <br />106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br />OBSERVATION: At the time of inspection, it could not be demonstrated (not necessarily documented) that <br />employees who handle hazardous waste were properly trained based on the following observations: <br />1. When asked how the facility manages absorbent that is used to clean-up a chemical spill in the facility, Chelsea <br />Harris, Customer Engagement Lead, stated that she did not know. According to Chelsea, facility managers/leads <br />are given annual hazardous waste training. <br />2. When asked how the facility cleans up a chemical spill in the facility, Marisa Perry, Associate Lead, stated that <br />depending on the consistency and size of the spill, the facility will sometimes add water to the spill, mop up the <br />spill, and go over the spill area with the floor sweeper, or the facility will use paper towels to absorb the spill and <br />place the used paper towels in a bag, along with the product container that spilled, and then into the appropriate <br />hazardous waste tote. According to the PetSmart Retail Hazardous Products Disposal Program Training, observed <br />in the PetSmart Hazardous Material Disposal Process Binder in the hazardous waste storage area, leaking or <br />spilled products are to be cleaned up using an absorbent, the used absorbent and any other clean up materials <br />used are to be placed into the re -sealable bag with the spilled product, or a separate re -sealable bag, and then <br />placed into the correct hazardous waste tote (i.e. Acid, Alkaline, Flammable, Oxidizer or Toxic) (see photos IMG <br />9419, 9425, 9434, 9437). <br />3. The only completed PetSmart Waste Storage Area Weekly Inspection Checklist found at the time of inspection <br />was dated 1/23/19 (IMG 9448). According to the PetSmart Retail Hazardous Products Disposal Program Training <br />document, the Waste Storage Area Weekly Inspection Checklist must be completed once a week by the Store <br />Manager, and must be maintained on file in the training and compliance binder in the store for 3 years (IMG 9425, <br />9438). <br />4. The PetSmart Waste Storage Area Weekly Inspection Checklist requires that the Store Manager ensure all <br />hazardous waste containers are properly closed, the generator (store) name is filled out on the hazardous waste <br />labels, and other hazardous waste requirements are fulfilled (IMG 9448). The following observations were made at <br />the time of inspection: <br />1) A Toxic Hazardous Waste tote, containing the bagged waste listed below, was observed missing a lid latch <br />on one side of the tote, causing the lid to not be properly closed (IMG 9384-95). <br />- a Bayer Seresto flea medication collar <br />- a box of Frontline Plus flea medication for dogs <br />- an open one gallon bottle of CHI Conditioner for dogs. <br />2) The Toxic Hazardous Waste tote mentioned above, which was labeled with an accumulation start date of <br />9/1/19, was observed with incomplete labeling (IMG 9384). The Toxic Hazardous Waste label was missing the <br />name of the generator. Chelsea wrote the name of the generator on the label at the time of inspection. <br />5. A 2 fl. oz. bottle of Tropiclean Fresh Breath brushing gel was found in the facility's dumpster (IMG 9409-15). <br />Marisa stated that the brushing gel might have been missed or was thought to be trash by one of the newer <br />employees, since toothpaste was not listed on the hazardous waste labels contents section, nor on the PetSmart <br />Potentially Hazardous Waste Management poster (IMG 9417).
