Brown Jr. <br />Govemor <br />Information for State ID Number Holders <br />PLEASE CAREFULLY READ ALL OF THE INFORMATION <br />IN THIS PACKET & KEEP FOR YOUR RECORDS <br />You have been issued a permanent State ID number for the generation, transportation and/or disposal of <br />hazardous waste. Your ID number must be used for all manifesting, record keeping and reporting <br />requirements- <br />As a holder of a permanent State ID number, you are required by law to respond to the annual business <br />information Verification Questionnaire and pay an annual ID number verification fee, if applicable. If you <br />generate hazardous waste at your business location and use manifests to ship it from your site to a <br />disposal facility, you may also be required to pay manifest fees. Please visit our web page for additional <br />information on ID numbers, manifests and fees at http://www.dtsc.ca.qov/IDManifest/index.cfm. <br />If you move your business or your business ownership changes you must get a new State ID number. <br />For information on how to update your information or apply for a new State ID number go to our website <br />at http://www.dtsc.ca.qov/IDManifest/index.cfm. <br />If you generate or produce five (5) or more tons of hazardous waste per calendar year, regardless of the <br />final disposition of the waste, you must apply for a hazardous waste account online by using eReg. To <br />apply, please visit the State Board of Equalization's website at <br />http://www.boe.ca.gov/sptaxprog/haz_sub tax_gen fee_reg.htm. Failure to pay fees due will result in <br />interest and a 10% penalty. See page 2 of this packet for specific details. <br />When starting a new business that handles hazardous waste or if you move your business, contact <br />your local environmental health department, environmental materials department or local Certified <br />Unified Program Agency and inquire what you must do at the local level. Any of these agencies, may <br />be at the county or city level, or may even be your local fire department. If you do not know which local <br />entity to contact, you may want to start at http://cersapps.calepa.ca.gov/Public/Directorv/ <br />DTSC's Hazardous Waste Tracking System (HVVTS) is available at www.hwts.dtsc.ca.00v. HWTS has <br />reports to find ID numbers, businesses, manifest data and waste produced in the State. <br />You can get fact sheets, news releases, regulations, public notices, information about managing <br />hazardous waste, compliance and enforcement, and pollution prevention at DTSC's website.
