State of California — Environmental Protection Agency Department of Toxic Substances Control <br />When you sign the manifest, you are certifying that the information on it is <br />correct, so take care in reviewing the manifest if someone else has filled it out for <br />you. After you have signed and dated item 15 of the completed manifest, the <br />transporter will sign and date item 17 and give you two copies. You are required <br />to send a legible photocopy of the manifest to DTSC within 30 days of each <br />shipment (Chapter 12, Section 66262.23, Title 22, CCR). <br />When the designated treatment, storage or disposal facility (TSDF) receives and <br />accepts your waste, their representative will sign and date item 20 on the <br />manifest. California TSDFs are required to send the completed signed copy of <br />the manifest to DTSC, and send a copy to the generator. You are required to <br />keep the generator copy and the signed copy from the California TSDF for at <br />least three years. (Chapter 12, Section 66262.40(a), Title 22, CCR). TSDFs in <br />other states are required to submit copies to DTSC when waste generated in <br />California is received out of state. <br />If you are a generator of greater than 1000 kg per month and you do not receive <br />a copy of the manifest signed by the TSDF representative within 45 days of the <br />date the waste was accepted by the initial transporter, you are required to file an <br />Exception Report with DTSC. If you are a generator of less than 1000 kg per <br />month and you do riot receive a copy of the manifest signed by the TSDF <br />representative within 60 days of the date the waste was accepted by the initial <br />transporter you are required to file an Exception Report with DTSC. Please send <br />a legible copy of the manifest with a cover letter explaining what you did to locate <br />the hazardous waste and the results of your efforts. Please send your Exception <br />Report to: DTSC Report Repository, Generator Information Services Section, <br />P.O. Box 806, R1-4, Sacramento, CA 95812-0806. A copy of the Exception <br />Report must be retained in your records for three years. (Chapter 12, section <br />66262.42(b), title 22, CCR) <br />MANIFEST INFORMATION <br />Please pay particular attention to some problems we have seen on manifests. <br />Generator Copies: <br />Mail a legible copy to P.O. Box 400 within 30 days of shipping the waste. <br />If the waste is being shipped to a California disposal facility, the generator <br />may submit Copy 2 of the manifest instead of a photocopy. <br />Legible Copies: Photocopy quality has been very poor and many of the <br />manifests are illegible. The photocopy must be legible; otherwise it cannot <br />be entered into our tracking system. <br />Only shipping descriptions are allowed in Item 9b on the manifest (no <br />ERG, profiles, etc.) <br />DTSC is providing information about top manifest offenders to local <br />enforcement agencies (CUPAs). <br />(02/16) STATEPKT Page 7
