The following conditions shall be met (3 <br /> by the facility as part of the annual operating permit. <br /> 1) A permit fee of$1,100.00 shall be paid upon issuance of the liquid waste permit_ <br /> 2) An annual operating permit fee of$500.00 shall be paid to the Environmental <br /> Health Department. (� <br /> 3) For the permitting year 2009, analytical testing for wastewater constituents and <br /> reporting to the EHD shall be required as part of an operational condition of the <br /> permit. The testing is required per San Joaquin County Onsite Wastewater <br /> Disposal Standards, Section 4.13., (San Joaquin County Ordinance Code <br /> Section 9-110.5). The monitoring frequency shall be bi-annual, commencing with <br /> the first quarter of January 1, 2009. The wastewater shall be sampled for the <br /> following constituents by an employee of a state-certified analytical laboratory <br /> and submitted to the EHD for review: <br /> a) TN - Total Nitrogen <br /> b) Nitrate <br /> c) Nitrite <br /> d) Ammonia <br /> e) TKN - Organic Nitrogen & Ammonia <br /> f) BOD - Biological Oxygen Demand <br /> g) TSS - Total Suspended So,ids <br /> h) pH <br /> i) COD - Carbonaceous Oxygen Demand <br /> 1 <br />
