Agency Contact Information <br />Local Fire Department 911 <br />EPA National Response Center (800)424-8802 <br />The report should be made as soon as possible so long as it does not impede immediate control of the release or <br />interfere with emergency medical measures and should include the following about the release or threatened <br />release: <br />• The exact location <br />• The name of the person that made the report <br />• The type and quantity of hazardous material(s) involved <br />• The potential hazard(s) presented (if known) <br />• Appropriate emergency response agencies and facility managers should also be notified. <br />d. Written Record: A written record of all verbal notifications should be prepared by the reporting <br />individual and provided to the owner.
