I closed violation 110 for the hazardous waste manifests. Violations 104 and 105 cannot be closed since you did not provide a photo of a complete modified contingency plan. Fill out and post the attached SQG contingency form or the equivalent. The emergency coordinator assignment must be "Emergency Coordinator", Paul Woolley, Store Manager, phone number. Include all elements on the attached form. Violation 605 cannot be closed since the composition of the hazardous wastes has not been added to the labels. For example: If flares are in the container add the word "flares" on the label. Violation 3030 cannot be closed since it was not mentioned in the information I've received. Provide a return to compliance statement and supporting documentation. A hazardous waste generator permit with my department (San Joaquin County Environmental Health Department) only authorizes your business to store and dispose of hazardous waste generated in the store. Please direct customers with hazardous waste to the San Joaquin County Household Hazardous Waste facility, 7850 RA Bridgeford St, Stockton, CA 95206, www.sacrecycle.org. West Marine is not an authorized collection point for hazardous waste received from customers unless the California Department of Toxic Substances Control has issued a permit for this activity.
