Permit Holder must submit a Waste Acceptance Agreement(Addendum A)and waste stream specific analytical results,upon <br /> request,for approval of each waste stream. EBMUD Resource Recovery Program will review the submittal and either approve or <br /> not approve the waste stream for receipt at the WWTP facility. Disposal may not proceed without prior approval from EBMUD. <br /> 2. Permit Holder,its Generators and Haulers shall meet discharge limits and acceptance as follows: <br /> a. Not discharge any hauled waste from a source that has not been approved on the Waste Acceptance Agreement,Addendum A. <br /> b. Not dispose any regulated radioactive waste,regulated PCBs,or hazardous waste,as defined in Section 25117 of the California <br /> Health and Safety Code or as defined by any other federal,state,or local statute or regulation,to EBMUD WWTP. <br /> c. Not dispose of any waste that may interfere with Treatment Plant Operations. <br /> d. Allow EBMUD staffto conduct random inspections and collect samples at the process generating waste origin and on the truck, <br /> I before and/or during disposal at the EBMUD WWTP, <br /> ' INSURANCE REOUfREMENTS <br /> Permit Holder shall and require its Haulers,Generators,Brokers,and Subcontractors to comply with the requirements outlined in the <br /> certifications following: <br /> 1. EBMUD Certification of Commercial General Liability Insurance,(Addendum B). <br /> 2. EBMUD Certification of Commercial Auto/Trucking Liability Insurance,(Addendum Q. <br /> 3. EBMUD Certification of Workers'Compensation Insurance,(Addendum D). <br /> HAULER AND SITE DISPOSAL CONDITIONS <br /> Permit Holder shall require the following: <br /> 1. Haulers tanker(s)to have an affixed EBMUD issued decal in order to enter the WWTP. <br /> 2. Hauling Company to submit contact information and drivers'name(s). <br /> 3. All drivers to schedule and conduct with EBMUD a site orientation at each disposal location. <br /> 4. Hauler to maintain appropriate and valid county and or state permit(s)for hauling liquid waste. <br /> 5. Hauler to comply with all applicable Cal OSHA requirements,including Title 8 of the California Code of Regulations-Section <br /> 3210(b)for driver fall protection. <br /> 6. Hauler to supply the equipment necessary to properly dispose of trucked waste including a back-up horn alarm safety device and <br /> fittings. <br /> 7. Hauler to comply with the EBMUD Procedures for the Delivery of Trucked Waste,(Addendum E). <br /> 8. Hauler to dispose all trucked waste at the designated disposal locations. <br /> 5/2/04 <br />
