TIWANA GAS & FOOD <br /> 1210 E HAMMER LANE SR0081496 <br /> STOCKTON , CA 95210 <br /> The San Joaquin County Environmental Health Department ( EHD ) has completed the review of <br /> the application for a permit to replace dispensers with conversion frames and convert the <br /> premium tank to a diesel , convert the 89 tank to 91 and cold start the monitoring panel on the <br /> underground storage tank ( UST) system at the above noted address . The permit has been <br /> approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br /> any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval prior <br /> to commencing work . All new addenda supersede the previous ones . <br /> 3 . Contact Paul Nso at (209 ) 468 - 3432 to arrange the following appointments ( a 48 - hour notice <br /> is required for inspection appointments ) . <br /> • Verification of the dispenser footprints contained by the under dispenser containment <br /> sumps ( UDCs) . <br /> • Visual inspection of the piping within the dispensers for any leaks . <br /> • Verification of the installation of the Bravo CONY- 380 conversion frame per <br /> manufacturer instructions . <br /> • Sensor functionality : Functionality of the monitoring system must be verified to ensure <br /> that monitoring has been restored to meet the requirements of Title 23 California Code <br /> of Regulations and the Health & Safety Code . A copy of the alarm history shall be <br /> printed prior to performing the " cold start" of the monitoring system and made available <br /> at the time of EHD inspection . <br /> • If concrete will be broken near the tank , piping or sumps a secondary containment test <br /> must be performed on the affected components once concrete is poured in accordance <br /> with the manufacturer' s guidelines and rest results submitted to the EHD within 30 days <br /> of testing . <br />
