This is a minor violation. 106 CCR 2632 (d ) (2 ) , 2634 (e) Failed to have an approved UST response plan . The response plan is not current on CERS . The response plan on site is dated 01 /23/2019 , while the one on CERS is dated 08/ 14/2014 . The current response plan must be uploaded to the California Environmental Reporting System (CERS) . Immediately log into CERS , upload the completed form , and submit for review by the EHD . Ensure a copy is available for inspection at any time . This is a minor violation. 201 CCR 2712 (b ) ( 1 -2 ) Failure to maintain monitoring , maintenance , and follow up action records . Maintenance and monitoring records for the last three years were not found on site . -Secondary containment testing results performed on 10/25/2018 were not found on site . These records shall be maintained on site for at least three years . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; (3) results of any visual observations ; (4) results of sample analysis performed a lab or in the field ; (5) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations . Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD This is a minor violation. 209 CCR 2716 (a-e) Failure to conduct the designated UST operator visual inspection at least once every 30 days . The designated operator failed to check and note all required items on the visual inspection reports . -The designated operator visual inspection reports of 3/6/19 , 2/6/19 , 1 /9/19 , 12 /12/18 , 11 /12/ 18 , and 10/17/18 state that the secondary containment testing was performed on 10/22/18 instead of 10/25/18 -The designated operator visual inspection reports of 5/31 /19 state that the secondary containment testing was performed on 10/13/19 instead of 10/25/18 -The 10/17 /18 inspection reports left blank section V . During the visual inspection , the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report . Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports . This is a minor violation.
