The annual operating permit is not issued to the current operator and there is no written agreement between the person who was issued the permit and the operator to monitor the UST system. If a permit is issued to a person other than the operator of the tank,that person shall enter into a written agreement with the operator of the tank to monitor the UST system. Immediately submit a copy of this written agreement to the EHD. Alternatively, log into CERS, upload the written agreement, and submit for review by the EHD. This is a minor violation. 103 HSC 25292.2,25299.30 Failure to submit and maintain complete and current financial responsibility documents. Financial responsibility documents have not been submitted to the EHD. The financial responsibility documents on CERS are dated 4/12/18. Current financial responsibility documents are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, and upload the required documents. This is a minor violation. 104 CCR 2711 UST operating permit application for facility and tank information not submitted or current. 1) UST Facility Operating Permit is not current in CERS.The California Department of Tax and Fee Administration (CDTFA, previously BOE)number listed on CERS is not associated with the new owner.Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. 2) UST Tank Information forms for the 87 and diesel product tanks are not current in CERS. Under tank configuration for the 87 and diesel product tanks,tanks are incorrectly stated to have compartments. Both of these tanks are standalone tanks.Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, update the required information, and submit for review by the EHD. This is a minor violation. 105 CCR 2632(d)(1), 2634(d)Failed to have an approved UST monitoring plan. The monitoring plan is not current and/or not approved by the EHD. On the monitoring plans for all tanks, under dispenser containment(UDC)monitoring,the following sections were incorrectly marked: -"Detection of Leak into UDC Triggers Audible and Visual Alarms" -"UDC Leak Alarm Triggers Automatic Pump Shutdown" -"Failure/Disconnection of UDC Monitoring System Triggers Automatic Pump Shutdown" -"UDC Monitoring Stops Flow of Product at Dispenser" The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a minor violation.
