The designated operator shall train facility <br /> employees for which he or she is responsible in the proper operation and maintenance of the UST system once <br /> every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br /> the training records to the EHD. <br /> This is a minor violation. <br /> 301 HSC 25292.1(a)UST system is not operated to prevent unauthorized release, including spills and overfills. <br /> Alarm logs and monthly inspections were not available for review.According to Mr. Khaial (the owner's cousin), the <br /> facility has not begun to conduct monthly inspections since they started operating about two months ago.The <br /> underground storage tank system shall be continuously monitored and checked for leaks or malfunctions. Monthly <br /> UST inspections must be made and a log of inspection results must be maintained. Immediately begin performing <br /> monthly inspections and maintain a log of inspection results for the UST system. <br /> This is a Class II violation. <br /> 320 CCR 2635, 2637.2,2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill <br /> prevention. <br /> Overfill prevention testing has not been conducted.This testing was required to be conducted by October 13, 2018. <br /> Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is <br /> conducted within required time frames. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by July 26, 2019. <br /> NOTE: According to Sonia Salazar, store manager, change of ownership took place about two months ago and <br /> the process escrow is not finalized yet. It was noted that prior to operation, this facility was closed for about two <br /> to three weeks. <br /> Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
