SAN JOAQUIN <br />- jU!'Ji <br />Environmental Health Department <br />AMENDW <br />11/26/2019 <br />RCRA Large Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />Banfield Pet Hospital #1188 <br />10520 Trinity Pkwy, Stockton <br />November 20, 2019 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />116 <br />CCR 66262.42 Failed to comply with uniform hazardous waste manifest exception requirements. <br />OBSERVATION: The facility did not have exception reports on site for the following Uniform Hazardous Waste <br />Manifests (UHWM): <br />- 000861604WAS (10/17/2019) (see photo IMG 9592) <br />- 011392442FLE (11/6/2018) (generator copy also not found on site) <br />- 010868634FLE (5/23/2018) (generator copy also not found on site) <br />- 011387069FLE (3/22/2018) (generator copy also not found on site) <br />REGULATION GUIDANCE: <br />(a) A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact <br />the transporter and/or the owner or operator of the designated facility to determine the status of the hazardous <br />waste. <br />(b) A generator shall submit an Exception Report to the Department if the generator has not received a copy of the <br />manifest with the handwritten signature of the owner or operator of the designated facility within 45 days of the date <br />the waste was accepted by the initial transporter. The Exception Report shall include: <br />(1) a legible copy of the manifest for which the generator does not have confirmation of delivery; <br />(2) a cover letter signed by the generator or the generator's authorized representative explaining the efforts taken to <br />locate the hazardous waste and the results of those efforts. <br />(d) Generators shall submit the exception report to the department at: DTSC Report Repository, Generator <br />Information Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br />CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests listed above and send a copy to <br />the San Joaquin County Environmental Health Department (EHD). If the destination facility signed manifests <br />cannot be found then prepare an exception report for each missing manifest as required by Title 22 California Code <br />of Regulations section 66262.42. The exception report shall include a legible copy of the manifest for which the <br />generator does not have confirmation of delivery and a cover letter signed by the generator or the generator's <br />authorized representative explaining the efforts taken to locate the hazardous waste and the results of those efforts. <br />DTSC address is DTSC Report Repository, Generator Information Services Section, PO Box 806, Sacramento, CA <br />95812-0806.
