Provide a corrective <br />action statement and supporting documentation to the San Joaquin County Environmental Health Department <br />(EHD) within 30 days. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />NOTES: <br />- The facility's hazardous waste generator status has been changed to RCRA Large Quantity, based on the <br />facility's 2019 manifests demonstrating the facility generates in any single calendar month, or accumulates at any <br />time, over 1 kilogram (2.2. pounds) of RCRA acute hazardous waste (i.e. waste code P042). The facility's <br />hazardous waste permit fees will increase due to this change in generator status. <br />- The inspection report has been amended by changing the violations to reflect the facility's hazardous waste <br />generator status change to RCRA Large Quantity. <br />- Please submit a statement to the EHD explaining which wastes Banfield generates that are P -listed acute <br />hazardous waste with P042 waste code. <br />- According to the Banfield Pet Hospital Waste Management Plan, Banfield disposes of all non -controlled <br />pharmaceutical waste, and only the controlled pharmaceutical waste listed on Appendix A of the plan, as <br />hazardous waste. Any other controlled pharmaceutical waste, not listed on Appendix A, are managed in <br />accordance with Banfield's Returning and Expired Drugs policy. Please submit a statement to the EHD <br />explaining if Banfield has made a determination as to which pharmaceutical wastes are RCRA vs non-RCRA, for <br />both the non -controlled and the controlled pharmaceutical wastes. <br />- The most recent hazardous waste manifest found at the time of inspection was 000861604WAS (10/17/2019). <br />A Small Quantity Hazardous Waste Generator inspection checklist was provided to the facility operator on the <br />day of inspection. The EHD has amended and written the RCRA Large Quantity Hazardous Waste Generator <br />report on 11/26/2019 which replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting <br />paperwork, by 12/26/2019. <br />Starting September 1, 2018, all in -office CERS help will be provided at EHD hourly rate ($152).
