Hazardous Waste ManagementSection <br /> ® Hazardous properties <br /> ® Specified places for information specific to each hospital ( listed below) <br /> At the time wastes begin accumulating in the container, the hospital must complete these elements on <br /> the label to comply with labeling requirements : <br /> ® Date when accumulation first began <br /> ® Name and address of the generator <br /> ® Identify the various contents in the Expired and Waste Pharmaceutical Containers ( i . e . , propofol, <br /> dropped pills, etc . ) <br /> A new label with a new accumulation date must be placed on the container after each pick up . <br /> B . Hazardous Waste Accumulation and Disposal <br /> The facility is a SQG of hazardous waste under California law, and hazardous waste can be stored on site <br /> for up to 180 days . However, Banfield has scheduled the waste disposal vendor, Stericycle , to have all <br /> hazardous wastes picked up on a quarterly basis . If additional pick up of waste is required due to a <br /> container filling before the scheduled pickup, contact Stericycle at 1.800-815 -3770 when the container <br /> is two thirds to three quarters full . <br /> C . Hazardous Waste Permits, Registration, and Licenses <br /> This facility has a California Hazardous Waste Identification Number and is permitted by the local CUPA, <br /> in accordance with California regulations . As the hospitals will unlikely generate more than 220 pounds <br /> of hazardous waste in a month, the hospital is considered a CESQG under federal regulations and a US <br /> EPA identification number is not required . The HSE group maintains the appropriate registrations and <br /> permits for the hospital . <br /> D . Hazardous Waste Manifests & Recordkeeping <br /> The transport and disposal of each load of hazardous waste removed from the hospital is documented <br /> by a manifest . A manifest will be provided by Stericycle upon pick up of hazardous wastes from the <br /> hospital . Banfield must retain one copy of the manifest in its Safety Binder and give the transporter the <br /> remaining . The disposal facility receiving the hazardous waste shipment must provide a final copy of the <br /> manifest to the generator within 30 days of delivery, which can be obtained from Stericycle' s website . <br /> The completed manifest must be retained by Banfield as a record for at least 3 years from the date the <br /> waste was accepted by the initial transporter. Stericycle will submit manifests to DTSC within 30 days, as <br /> required . <br /> Additionally, waste characterization documentation is required to be maintained for at least 3 years <br /> after the last disposal event . In the event this information is requested , it can be obtained from the HSE <br /> group . <br /> E . Hazardous Waste Inspections <br /> All storage of hazardous waste at this hospital is indoors in the maintenance area or areas where wastes <br /> are generated . These areas must be inspected on a weekly basis .
