STOCKTON August 10, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 111 CCR 66262.42(a)(c)(d) Failed to comply with uniform hazardous waste manifest exception requirements. <br /> OBSERVATION: The facility did not have exception reports on site for the following Uniform Hazardous Waste <br /> Manifests(UHWM): <br /> -013123980FLE(11/11/2019) <br /> -010788843FLE(12/07/2017) <br /> REGULATION GUIDANCE: <br /> (a)A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br /> of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact the <br /> transporter and/or the owner or operator of the designated facility to determine the status of the hazardous waste. <br /> (c)A hazardous waste generator who does not receive a copy of the manifest with the handwritten signature of the <br /> owner or operator of the facility to which the generator's waste is submitted,within 60 days from the date that the <br /> hazardous waste was accepted by the initial transporter, shall submit to the department a legible copy of the <br /> manifest,with some indication that the generator has not received confirmation of delivery. <br /> (d)Generators shall submit the information to the department at: DTSC Report Repository, Generator Information <br /> Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br /> CORRECTIVE ACTION: Locate a copy of the destination facility signed manifests and send a copy to the San <br /> Joaquin County Environmental Health Department(EHD). If the destination facility signed manifests cannot be <br /> found then send to DTSC a copy of each generator signed manifest and a statement that the designated facility <br /> signed copy was not received by the generator. DTSC address is DTSC Report Repository, Generator Information <br /> Services Section, PO Box 806, Sacramento, CA 95812-0806. Provide a corrective action statement to the EHD <br /> within 30 days. <br /> This is a Class II violation. <br /> 303 CCR 66262.34(4)(2) Failed to maintain all communication or alarm systems, spill control, or decontamination <br /> equipment. <br /> OBSERVATION: <br /> 1. Two bottles of sterile eye and skin wash, observed on the counter next to the empty Emergency Eye and Skin <br /> Wash station in the Printer Center, are expired as of 8/18/2012. <br /> REGULATION GUIDANCE:All facility communications or alarm systems, fire protection equipment,spill control <br /> equipment, and decontamination equipment,where required, shall be tested and maintained as necessary to assure <br /> its proper operation in time of emergency. <br /> CORRECTIVE ACTION: Bring the Emergency Eye and Skin Wash station in the Printer Center into proper working <br /> order by repair, replacement or testing.
