Test results had <br /> missing components : <br /> Type of action was incorrectly stated to be 36 month test . <br /> Section I : <br /> Missing CERS ID <br /> Section II <br /> Missing contractor ' s license # <br /> Section VII : <br /> missing manufacturer of the product piping . <br /> Test method used : ensure that manufacturer guidelines are available in order to use industry code or engineering <br /> standards . <br /> Product piping containment testing training and certifications ( List applicable certifications ) and INCON level 4 <br /> # 1005763709 has been provided however this certification cannot be used to test piping under pressure . <br /> interstitial communication verification was left blank <br /> Furthermore, test procedures were not found attached to the test results . <br /> Please make the required corrections and submit test results to ust@slgov . org and reach out to Paul Nso at 209 -468 - <br /> 3432 .
