The owner/operator failed to sign the following designated inspection report under section VI within 48hours. The Designator operator(DO) inspection report of 8/21/19 was signed on 9/3/2019, 11 days late. The DO report of 7/22/19 was signed on 9/3/19, 40days late The DO report of 6/21/19 was signed on 9/3/19 ,72days late The DO report of 5/22/19 was signed on 9/3/19, 102 days late The DO report of 3/22/19 was signed on 3/25/19 , i day late The DO report of 2/21/19 was signed on 2/27/19 , 4days late. The DO report of 11/20/18 was signed on 11/28/18, 6days late. The owner/operator acknowledgment of compliance is required within 48 hours of the date of the designated UST operator inspection report. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a minor violation. 319 CCR 2635, 2637.1, 2665, 2712 No spill bucket, failed to contain 5 gallons, no means to keep it empty, or failed to be tested. The spill container when tested did not hold 5 gallons. All USTs shall be equipped with a spill container that collects any hazardous substances spilled during delivery operations, that has a minimum capacity of five gallons and a drain valve that allows drainage of the collected spill into the primary container or provide a means to keep the container empty. Immediately contact a properly licensed, trained, and certified contractor to install an approved spill container under permit and inspection of the EHD. If a spill container can't be installed immediately, there is a possibility that the UST system may be red tagged to prevent fuel inputs. This is a minor violation. 320 CCR 2635, 2637.2, 2665, 2712 Failure to comply with the installation, inspection, or record keeping of the overfill prevention. Overfill prevention testing has not been conducted. This testing was required to be conducted by October 13, 2018. Immediately schedule this testing and provide 48 hours notification to the EHD. Ensure all future testing is conducted within required time frames. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by October 26, 2019.
