In an attempt to <br /> satisfy your inspection, the panel name was change from TLS350 to Simplicity in CERS. You requested the UDC panel <br /> model and manufacturer for monitoring which DOES NOT exist as the UDC utilize a float and chain mechanism. <br /> Violation#121—No response was sent for the 8-28-18 inspection.All issues corrected. <br /> Violation#202—The Facility Employee Training Certificate does include both the date of training and the date the <br /> employee took responsibility as an employee. All employees received refresher training within the annual period. <br /> Violation#209—The Designated Operator did note and check all items on the visual inspection reports. The May,June <br /> and July reports did not require any follow-up or response and DID NOT require a signature for any follow-up. The July <br /> report only requested copy be on site for the testing after completion. The testing was completed after the monthly <br /> inspection. On-site manager now reviews all reports. <br /> Violation#301 &#314—Secondary containment for the fuel filters is under review and we are seeking estimates to <br /> remedy the situation. <br /> Sincerely, <br /> Amin, Sardar <br />
