The monitoring plan is not current and/or not approved by the EHD. The monitoring plans for both tanks do not include the UDC sensor manufacturer or model. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. 118 CCR 2715(a)(1)(A), 2715(a)(2) Failure to submit the "Underground Storage Tank Statement of Understanding and Compliance Form". The Underground Storage Tank Statement of Understanding and Compliance Form was not found in CERS. This form must be uploaded to the California Environmental Reporting System (CERS). Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the Underground Storage Tank Statement of Understanding and Compliance Form for this facility. This is a minor violation. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. Overfill prevention test results for 9/26/2018 were not found on site. These records shall be maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site. This is a repeat violation, Class II. 202 CCR 2715(c)(3), 2715(c)(4) Failure to maintain the "Facility Employee Training Certificate" on-site (after October 1, 2018). -The Facility Employee Training Certificates contained no CERS ID, no phone number, and the designated operator expiration date was incorrect. -The designated employee training conducted on 12/17/18 and 11/21/18 were on an outdated form. All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificates to contain the accurate information and submit copies of the updated forms to the EHD. This is a minor violation.
