All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. Update the Facility Employee Training Certificates to contain the accurate information and submit copies of the updated forms to the EHD. This is a minor violation. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 209 CCR 2716(a -e) Failure to conduct the designated UST operator visual inspection at least once every 30 days. The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -Overfill prevention test date was left blank. -The NA answers are not included in Section III. -Section VI was left blank. -The question regarding UDC sensor locations was left blank. - CERS ID was left blank. -Monitoring system certification test date was incorrect. - Spill Container test date was incorrect. - Secondary containment test date was incorrect. - Section VI was not dated. - The designated operator signature was left blank. -The reports indicate that containment sumps are being inspected by designated operator. The question regarding containment sump sensor location was left blank. During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation.
