Henry , <br /> PERMIT/SERVICES <br /> The purpose of this letter is to respond to the fifteen violations that were noted during the June <br /> 131 2017 Hazardous Waste inspection that occurred at the Stockton Medical Office . <br /> Violations : <br /> # 112 - CCR 66262 . 20 & #203 - HSC 25189 . 5 ( c ) : Kaiser requires that expired pharmaceuticals <br /> be managed as hazardous waste and shipped under manifest to a licensed hazardous waste <br /> transfer, treatment , storage or disposal facility . Expired pharmaceuticals are not to be sent to <br /> Inmar or any other facility unless the facility is licensed as a hazardous waste transfer, <br /> treatment , storage or disposal facility . Affected employees are being retrained on this <br /> requirement . For additional information , see response to # 118 regarding training . <br /> Warfarin waste was picked up .
