Date: <br /> Manufacturer training: <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> NOTES <br /> On site for the following inspections: <br /> -Verified a copy of the Health and Safety Plan (HASP)was on the jobsite for review. Discussed the Health and Safety Plan with <br /> Belshire, Speelman Excavation,Antea Group and TCCI onsite. <br /> -Verified that a copy of the approved fire permit was available onsite at the time of inspection. <br /> -Verified that all 4 tanks were inerted with dry ice after the triple rinsing process took place at the 30-pounds of dry ice per <br /> 1,000-gallon volume rate.According to the Airgas receipt observed onsite, 1,400 pounds of dry ice was used to complete this <br /> process. <br /> -Verified the hazardous waste manifest for the rinsate that was generated. <br /> -Witnessed lower explosion level (LEL)readings of the bottom, middle, and top section of inside each tank.The LEL reading <br /> for all 4 tanks was 0% LEL.-Verified and witnessed the marking of the tank ID numbers on all 4 tanks.The tanks were taken <br /> out of the hole and hauled in the following order: <br /> -Tank#:TA0134907-10, 000 gallon regular unleaded gasoline tank <br /> -Tank#:TA0134908-10, 000 gallon regular unleaded gasoline tank <br /> -Tank#: TA0134909-10, 000 gallon regular unleaded gasoline tank <br /> -Tank#: TA0134910-10, 000 gallon premium unleaded gasoline tank <br /> -Verified EHD's Underground Storage Tank Disposition Tracking Record Sheet was accompanying each tank as they were <br /> being hauled away. <br /> -Witnessed soil sampling from below all removed tanks, piping,stockpile soil and new tank pits. (1 sample=6 vials and 2 jars) <br /> The soil samples beneath the tanks and piping was obtained with an excavator. Samples were collected at least 2 feet below <br /> the native soil. Groundwater and slap was not observed during excavation.-Tank soil samples:Two samples, one at each end <br /> of the tank was taken from beneath each tank. (Total of 8 tank soil samples were taken). The soil samples obtained from the <br /> tanks were described to be a silty/sandy soil. The photoionization detector(PID)readings for these tank soil samples were <br /> O.Oppm. <br /> -Piping soil samples: One soil sample was obtained at intervals of 15 feet. Samples obtained after the removal of product lines <br /> and dispensers were noted to smell like gasoline with a soil color of greyish/green. Sample PL1A:Two samples were collected <br /> at the elbow of the product lines. PID readings were obtained from the product lines at a closer interval.The surface soil <br /> sample from this area had a PID reading of 36.3 ppm and 3 PID readings of 0.3, 0.7, and 0.7 ppm. Sample PL8 by UDC3/4 <br /> had a PID reading of 20.2 ppm. Sample PLEA had PID reading of 39.4 ppm. Sample PL9A had a PID reading of 36.5 ppm. <br /> The rest of the samples taken from the piping had a PID reading of O.Oppm.-Soil stockpiled from excavation was sampled. <br /> Two samples(one on each end)were taken had a PID reading of O.Opm. <br /> -Eight samples were taken from the new tank pit.
