According to Mr. Yiamkis, he had taken a sample earlier and forgot to close a valve, which pushes the gasoline to the station, causing a leak. The gasoline had filled a sink and was overflowing onto the concrete underneath and into the gravel/dirt secondary containment. Mr. Yiamkis shut off the valve and cleaned the leak area at the time inspection; clean up was not witnessed. -A forklift in the garage was observed leaking oil onto the floor. No absorbent or other responses were observed to address the oil leak and no maintenance records were available for attempted repairs for the leak. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately fix all leaking equipment, clean all spill areas, including stained soil, and manage according to Title 22 hazardous waste regulations. Submit a statement and supporting documentation explaining how these wastes were managed. Immediately close these containers, or place the waste in properly closed containers, and ensure all hazardous waste containers are closed when not adding or removing waste. A blue poly drum of gasoline contaminated debris was observed in the garage with an accumulation start date of 6/23/15. Facilities who generate less than 1000 kg of hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. Immediately contact a licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. This is a Class II violation.
