It states specifically what 'uniform hazardous waste manifest' I am looking for. Page 5 of your attachment includes manifest 010853096JJK but does not have the signature at the very bottom on line item 20 by the designated facility confirming receipt of the oily solid waste.The signature copy is what I need to confirm your 3 missing manifest have been received by the designated facility. Please find the 3 manifest and include a copy completed "Return to Compliance Certification" form. Try contacting the transporter or the designated facility if they would happen to have a copy.
