The permit has <br /> been approved with the following conditions : <br /> 1 . Contact the local fire department and the San Joaquin Valley Air Pollution Control District <br /> for any applicable permits and requirements . <br /> 2 . Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work . All new addenda supersede the previous ones . <br /> 3 . Provide written notification to ust (a) sigov . org or contact Shewit Woldearegay at (209 ) 468 - <br /> 0216 to arrange the following appointments ( a 48- hour notice is required for inspection <br /> appointments ) . <br /> a ) Corrosion protection : Verify corrosion protection for direct bury spill bucket prior to <br /> backfilling . <br /> b) Spill bucket test : Verify minimum 5 -gallon capacity and pass a one hour hydrostatic <br /> test ( or equivalent) . Proper drainage must also be verified . <br /> c) Overfill prevention verification : Measurement of drop tubes to verify 95 % tank <br /> volume shut off. Verification of flapper functionality . Ensure that all pertinent tank <br /> charts and calculations are available for review at the time of inspection . <br /> d ) Since concrete will be broken near the diesel tank , a secondary containment test <br /> must be performed on the diesel tank once concrete is poured in accordance with <br /> the manufacturer' s guidelines . <br /> Please be aware that the facility cannot receive fuel in the 87 and 91 tank prior to the <br /> EHD inspection and approval . <br /> 4 .
