Please find below a list of the violations noted at the referenced facility <br /> and Pilot Flying J's (PFJ) responses. <br /> UST Program Inspection <br /> 1. The designated operator failed to document all the alarms from the attached alarm <br /> history on the 1-23-14 and 7-24-13 designated operator monthly inspection reports <br /> and failed to document that they were responded to appropriately. The missing <br /> alarms include: 12-20-13 L2 87 T1 annular low liquid alarm and 7-20-13 L7 87 T4 stp <br /> sump sensor fuel alarm. During the monthly inspection, the designated operator <br /> shall review the alarm history for the previous month, check that each alarm was <br /> documented and responded to appropriately, and attach a copy of the alarm history <br /> with documentation taken in response to any alarms to the monthly report. Ensure <br /> that designated operators performing monthly inspections at this facility are including <br /> all of the required information on the reports. <br /> Although the L2 87 T1 annular low liquid alarm from 12-20-13 was not noted on the <br /> first page of the designated operator monthly inspection report, it was marked on the <br /> alarm history tape during the inspection and the appropriate PFJ response was <br /> verified.
