submit a legible copy of the missing manifest, with some indication that the generator <br /> has not received confirmation of delivery. Immediately locate a copy of the missing <br /> manifests or prepare and submit the required information to DTSC. <br /> Please find enclosed a copy of the referenced manifests. When hazardous waste is <br /> removed from a PFJ location, a copy of the manifest is placed in a Hazardous <br /> Materials binder at the facility. The final copy of the manifests are sent to the <br /> corporate office for review by the designated facility, and subsequently sent to the <br /> store as soon as possible to be placed in the Hazardous Materials binder at the <br /> facility. Copies of these manifests have been sent to the facility and PFJ has <br /> reviewed the proper procedure for maintaining manifests onsite. <br /> 2. An emergency coordinator and modified contingency plan information is lacking. <br /> There must be at least one emergency coordinator on site or on call to coordinate <br /> emergence response measures, and the following information must be posted by a <br /> phone: the name and phone number of the emergency coordinator; location of fire <br /> extinguishers, spill control equipment, and if present, fire alarm; and the phone <br /> number of the fire department, unless facility has a direct alarm. Immediately <br /> appoint an emergency coordinator and post the required information by a phone. A <br /> form is provided that can be used for this purpose. <br /> The Consolidated Emergency Response/Contingency Plan was uploaded to CERS <br /> and accepted by the EHD in February 2014, and a copy is enclosed for your review. <br /> Additionally, the form with emergency coordinator contact information, location of fire <br /> extinguishers and spill control equipment, and emergency phone numbers has been <br /> completed and posted by the phone. A copy is enclosed for your review. <br /> Having corrected the violations noted during the inspection, it is our understanding that this <br /> matter is closed and that PFJ is in full compliance with the requirements of the EHD.
